COVID-19 Questionnaire
All players and volunteers must complete the following questionnaire before coming to the field.
Temperature check will be performed at the field.
DO NOT ATTEND IF:
- You/your player answers YES to any of the questions below
- You/your player are feeling unwell for any reason
- You/your player have been in close contact* with someone who is feeling unwell with COVID-19 symptoms

If player/coach tests positive for COVID-19 or has been exposed to a COVID-19 positive individual:
Players: IMMEDIATELY notify your COACH
Coaches: IMMEDIATELY notify the REGIONAL COMMISSIONER at [email protected].
Player and coach confidentiality will be maintained, and the Return to Play protocol must be followed
*Close contact: longer than 15 minutes within 6 feet of someone
NOTE: You do not need to bring the completed questionnaire with you to the field
